The Silverlake community pool located at 2715 Southwyck Parkway will open Saturday, May 1, 2010.
FOR LIABILITY PURPOSES WE ASK THAT EACH RESIDENT RETURN THE POOL INDEMNIFICATION FORM TO PCMI ON AN ANNUAL BASIS. THE ASSOCIATION MAY EXCERCISE THE RIGHT TO DEACTIVATE YOUR ACCESS CARD IF THIS FORM IS NOT RETURNED.
IF YOU HAVE ALREADY RECEIVED YOUR ACCESS CARD FROM PREVIOUS YEARS OR ARE NOT REQUESTING ADDITIONAL CARDS, PLEASE DO NOT RETURN THE ACCESS CARD REQUEST FORM.
Enclosed are the 2010 Registration and Indemnification Forms which must be filled out by all residents in order to receive pool and/or tennis court access cards. Please complete these forms and return them to PCMI so on access card may be sent to you. There will be no charge for the first (1) access card issued. Each additional access card or replacement card is available at a cost of $20.00 per card. You will not be required to obtain another card unless you wish additional cards for your individual family members or in the event you lose the complimentary card.

Your information will be entered into the access card system and the card will be mailed to your residence. You may not pick up the access card in person.
You will be required to show photo identification when using the card at the pool facility. You may use any photo identification card or your driver's license reflecting your local address to gain admittance.
LIFEGUARDS WILL NOT PERMIT ACCESS TO ANYONE AT THE
2715 SOUTHWYCK PARKWAY POOL FACILITY
WITHOUT THE PROPER ACCESS CARD AND PHOTO IDENTIFICIATION.
** NO EXCEPTIONS **
The Association may suspend use of the common area including but not limited to the pool and tennis courts during any period for which an assessment in delinquent. If there is a DELINQUENCY ON YOUR ACCOUNT, you will not be eligible to receive an access card until the delinquency is current.
Access may be denied for deed restriction violations.
